AMQAS 2012 entry form

	Adelaide Convention Centre, North Terrace, Adelaide, SA. One form is to be completed for each quilt submitted. Please read this form carefully and fill in every section. A full size and close-up photograph of each quilt must accompany the entry form. A payment fee of $15- per quilt or $7.50 per quilt for AMQA Members must accompany this entry form.
